Para tratar de confirmar o falsar dichas hipótesis se ha analizado acústicamente una amplia muestra de vocales de las dos lenguas mencionadas. La discusión de los resultados de este análisis interlingüístico permite extraer algunas conclusiones potencialmente interesantes para la teoría fonético-fonológica. For this purpose we analyzed the spectral information of the vocalic fillers by estimating their first two formant frequencies as well as their duration properties. The vocalic fillers are taken from a large corpus of European Portuguese broadcast news' speech. We also compared the vocalic fillers with lexical vowels possessing similar timbre. No formant variation trend was attained for the vocalic fillers and a great overlap of formant values is observed. METHODS: A review of prospectively collected clinical and pathology data was performed for all patients who underwent BCS between 2006 and 2010 through a single cancer centre. An involved margin was defined as {\textless} 1 mm clearance of invasive or in-situ breast cancer. RESULTS: 1667 patients underwent BCS for invasive and/or in-situ disease, of whom 114 underwent RE. A total of 170 involved margins were identified: most commonly the anterior (52 margins) followed by the posterior (39 margins) and inferior (31 margins) margin. Patients with IAM were more likely to have grade 3 invasive disease (p = 0.0323) but less likely to have residual disease found at re-excision (2/49 vs. 32/101 margins, p = 0.0033); there were no differences when in-situ characteristics were compared. We describe the absolute magnitude of these reductions according to various prognostic and other patient characteristics, and relate the absolute reduction in 15-year risk of breast cancer death to the absolute reduction in 10-year recurrence risk. METHODS: We undertook a meta-analysis of individual patient data for 10,801 women in 17 randomised trials of radiotherapy versus no radiotherapy after breast-conserving surgery, 8337 of whom had pathologically confirmed node-negative (pN0) or node-positive (pN+) disease. FINDINGS: Overall, radiotherapy reduced the 10-year risk of any (ie, locoregional or distant) first recurrence from 35·0\% to 19·3\% (absolute reduction 15·7\%, 95\% CI 13·7-17·7, 2p{\textless}0·00001) and reduced the 15-year risk of breast cancer death from 25·2\% to 21·4\% (absolute reduction 3·8\%, 1·6-6·0, 2p=0·00005). In women with pN0 disease (n=7287), radiotherapy reduced these risks from 31·0\% to 15·6\% (absolute recurrence reduction 15·4\%, 13·2-17·6, 2p{\textless}0·00001) and from 20·5\% to 17·2\% (absolute mortality reduction 3·3\%, 0·8-5·8, 2p=0·005), respectively. In these women with pN0 disease, the absolute recurrence reduction varied according to age, grade, oestrogen-receptor status, tamoxifen use, and extent of surgery, and these characteristics were used to predict large (≥20\%), intermediate (10-19\%), or lower ({\textless}10\%) absolute reductions in the 10-year recurrence risk. Absolute reductions in 15-year risk of breast cancer death in these three prediction categories were 7·8\% (95\% CI 3·1-12·5), 1·1\% (-2·0 to 4·2), and 0·1\% (-7·5 to 7·7) respectively (trend in absolute mortality reduction 2p=0·03). In the few women with pN+ disease (n=1050), radiotherapy reduced the 10-year recurrence risk from 63·7\% to 42·5\% (absolute reduction 21·2\%, 95\% CI 14·5-27·9, 2p{\textless}0·00001) and the 15-year risk of breast cancer death from 51·3\% to 42·8\% (absolute reduction 8·5\%, 1·8-15·2, 2p=0·01). Overall, about one breast cancer death was avoided by year 15 for every four recurrences avoided by year 10, and the mortality reduction did not differ significantly from this overall relationship in any of the three prediction categories for pN0 disease or for pN+ disease. INTERPRETATION: After breast-conserving surgery, radiotherapy to the conserved breast halves the rate at which the disease recurs and reduces the breast cancer death rate by about a sixth. These proportional benefits vary little between different groups of women. By contrast, the absolute benefits from radiotherapy vary substantially according to the characteristics of the patient and they can be predicted at the time when treatment decisions need to be made. L'idea che muove i lavori cui facciamo riferimento (Llisterri et al., 2005; Alfano, 2006; Alfano et al., 2007) nasce dall'intento di valutare il peso dei correlati acustici dell'accento lessicale in percezione. Tali indagini sono state condotte partendo da esperimenti percettivi realizzati con stimoli (parole e non parole in isolamento) ottenuti in seguito alla manipolazione sistematica di durata e frequenza fondamentale. A dispetto delle rilevanti proprietà strutturali condivise dai due sistemi linguistici, in italiano la durata risulta giocare un ruolo dominante (Alfano, 2006), mentre in spagnolo la f0 si delinea come il parametro responsabile della percezione dell'accento, se manipolato però insieme alla durata o all'intensità (Llisterri et al., 2005). I risultati mostrano, per ambedue le lingue, la natura complessa del processo di percezione, ma ancor più articolata si delinea l'analisi dei dati relativi allo stesso protocollo sperimentale impiegato in un lavoro linguisticamente incrociato, con stimoli in italiano e parlanti nativi di spagnolo (Alfano et al., 2007). Tale esperimento era stato messo a punto per capire se, ed eventualmente in che modo, la strategia percettiva dipende dalla lingua madre; i soggetti spagnoli paiono reagire all'ascolto di input in italiano diversamente da come fanno nella propria lingua. Mentre, infatti, in spagnolo si mostrano del tutto insensibili alla sola alterazione del fattore temporale, in italiano reagiscono agli stimoli con la durata alterata presentando evidenti difficoltà, sia nell'identificazione che nella discriminazione. Un'analisi del grado di coerenza delle risposte in relazione alla tipologia di modifica mostra, in conseguenza della manipolazione della durata, una situazione fortemente problematica. Data la peculiarità del comportamento dei soggetti in risposta all'alterazione di questo parametro, ci è parso necessario effettuare un'analisi delle caratteristiche acustiche di durata vocalica in italiano e in spagnolo. Presentiamo, dunque, un'analisi contrastiva della durata vocalica nei corpora impiegati negli esperimenti rispetto a: a) posizione della vocale, b) sua tonicità/atonicità, c) profilo accentuale della parola. Inadequate margins may result in high local recurrence, and excessively large resections may lead to poor cosmetic outcome without oncologic benefit. METHODS: A comprehensive search for published trials that examined outcomes after adjuvant RT after BCS for DCIS was performed using MEDLINE and cross referencing available data. Reviews of each study were conducted, and data were extracted. Primary outcome was ipsilateral breast tumor recurrence (IBTR) related to surgical margins. RESULTS: A total of 4,660 patients were identified from trials examining BCS and RT for DCIS. Patients with negative margins were significantly less likely to experience recurrence than patients with positive margins after RT (odds ratio [OR] = 0.36; 95\% CI, 0.27 to 0.47). A negative margin significantly reduced the risk of IBTR when compared with a close (OR = 0.59; 95\% CI, 0.42 to 0.83) or unknown margin (OR = 0.56; 95\% CI, 0.36 to 0.87). When specific margin thresholds were examined, a 2-mm margin was superior to a margin less than 2 mm (OR = 0.53; 95\% CI, 0.26 to 0.96); however, we saw no significant difference in the rate of IBTR with margins between 2 mm and more than 5 mm (OR = 1.51; 95\% CI, 0.51 to 5.0; P {\textgreater} .05). CONCLUSION: Surgical margins negative for DCIS should be obtained after BCS for DCIS. SUBJECTS: We retrospectively examined 1016 patients after intraoperative FSA during breast-conserving therapy for breast cancer operated between 1995 and 2001 at the Medical University Vienna. RESULTS: Only 9\% of all patients had to undergo a two-stage operation due to a false negative intraoperative FSA result. The annual local recurrence rate was 1.2\% in all patients with no difference between one- and two-stage operated patients. In situ and pT1 lesions were similarly distributed between one-stage and two-stage operated patients. The use of neoadjuvant therapy and stereotactic biopsy (reflecting non-palpable lesions and microcalcifications) were significantly predictive for a false negative FSA result. The use of a preoperative core biopsy, however, reduced the necessity of performing a two-stage operation. CONCLUSION: Our study demonstrates that FSA leads to a low rate of two-stage operations. Small lesions and microcalcifications as well as the occurrence of intraductal cancer cells and neoadjuvant therapy increased while preoperative core biopsy reduced the false negative rate of FSA. Hypothesis: Individual parameters will show less SD when grouping by speaker/session; vowel quality parameters, when grouping by phoneme. F1 and F2 are timbre‐dependent parameters. F3 and F4 are speaker‐dependent parameters. F0 has characteristics of both. The most variable parameter is F2. The opposite is F4. No significant differences grouping by session or by speakers in none of the parameters. F0 has the highest variability between vowel qualities, even if stressed and unstressed vowels are separated. METHODS: A total of 1851 women for whom follow-up data were available and nodal status was known underwent randomly assigned treatment consisting of total mastectomy, lumpectomy alone, or lumpectomy and breast irradiation. Kaplan-Meier and cumulative-incidence estimates of the outcome were obtained. RESULTS: The cumulative incidence of recurrent tumor in the ipsilateral breast was 14.3 percent in the women who underwent lumpectomy and breast irradiation, as compared with 39.2 percent in the women who underwent lumpectomy without irradiation (P{\textless}0.001). No significant differences were observed among the three groups of women with respect to disease-free survival, distant-disease-free survival, or overall survival. The hazard ratio for death among the women who underwent lumpectomy alone, as compared with those who underwent total mastectomy, was 1.05 (95 percent confidence interval, 0.90 to 1.23; P=0.51). The hazard ratio for death among the women who underwent lumpectomy followed by breast irradiation, as compared with those who underwent total mastectomy, was 0.97 (95 percent confidence interval, 0.83 to 1.14; P=0.74). Among the lumpectomy-treated women whose surgical specimens had tumor-free margins, the hazard ratio for death among the women who underwent postoperative breast irradiation, as compared with those who did not, was 0.91 (95 percent confidence interval, 0.77 to 1.06; P=0.23). Radiation therapy was associated with a marginally significant decrease in deaths due to breast cancer. This decrease was partially offset by an increase in deaths from other causes. As in previous research, English monolinguals showed rate-dependent effects on their VOT productions: VOT increased as speaking rate decreased. Spanish monolinguals showed a large effect of speaking rate on the duration of prevoicing of the voiced stops. However, they showed only a small effect of rate on the VOT of their voiceless stops. The bilinguals produced VOT values in each language that were nearly identical to their monolingual counterparts. The results from this study indicate that short-lag stops experience minimal variation as a function of speaking rate regardless of the other contrasting phonetic categories within a particular language. Meta-analysis may assist in understanding the impact of these differences on survival. PURPOSE: To evaluate the possible variations of the relative efficacy of BCT and mastectomy in terms of overall survival according to tumor size, nodal status, and use of adjuvant radiation therapy. METHODS: The most recent published results and, where available, updated patient-level data from randomized controlled trials of BCT and mastectomy for early-stage breast cancer were combined in a meta-analysis using a random effects model. Pooled survival rates and odds ratios were generated according to subgroups of nodal status and tumor size. Five- and 10-year odds ratios were also determined according to adjuvant radiation protocol. RESULTS: The pooled odds ratio comparing 10-year survival for BCT and mastectomy was 0.91. The odds ratios comparing the two treatment regimens were not significant after grouping according to tumor size and nodal status. When more than 50\% of node-positive patients in both the mastectomy and BCT arms received adjuvant radiation, both arms had similar survival rates. When less than 50\% of node-positive patients in both arms received adjuvant nodal radiation, the odds ratio was 0.69, and patients receiving BCT had a survival advantage. CONCLUSIONS: Patients allocated to BCT have survival rates at least as high as patients allocated to mastectomy. When all protocols were combined, nodal status and tumor size did not significantly alter the relative survival rates. However, under some conditions, particularly for node-positive patients, BCT may confer a relative survival advantage over mastectomy. Acoustical analysis of the recorded speech revealed the incidence of velarized (typical of Catalan) and non-velarized (typical of Spanish and French) lateral consonants. Results indicate that the production of lateral consonants in French tended to be the non-velarized variety found in French and Spanish.
