In most women with breast cancer, breast-conserving surgery plus local radiotherapy reduces the risk of recurrence in the ipsilateral breast and results in long-term survival similar to that after mastectomy.1–4 Adjuvant treatment with tamoxifen also reduces the risk of local relapse.5,6 Since mammographic screening is capable of detecting relatively small tumors and salvage therapy is successful in many women who have a relapse in the breast,3,7 there is interest in identifying patients at low risk who could avoid radiotherapy.
